Meet the HFACS, Inc. Team
Dr. Scott A. Shappell
Co-Founder
Dr. Scott A. Shappell is a Professor of Industrial Engineering at Clemson University. Before joining the faculty at Clemson, Dr. Shappell was the Human Factors Research Branch Manager at the Civil Aerospace Medical Institute. In addition, he has served over 16 years in the U.S. Navy as an Aerospace Experimental Psychologist He has published and presented well over 200 papers, books, and presentations in the fields of accident investigation, system safety, spatial disorientation, sustained operations and fatigue. Dr. Shappell received a B.S. in psychology (1983) from Wright State University graduating Summa Cum Laude with honors in psychology and a Ph.D. in Neuroscience from the University of Texas Medical Branch in 1990.
Dr. Douglas A. Wiegmann
Co-Founder
Dr. Douglas A. Wiegmann is an Associate Professor of Industrial and Systems Engineering at the University of Wisconsin. Before joining the faculty in Madison, Wisconsin he was a National Institutes of Health Roadmap Scholar at the Mayo Clinic College of Medicine where he also served as the Director of Human Factors and Patient Safety Research within the Division of Cardiovascular Surgery at Mayo. A private pilot, Dr. Wiegmann has also been an associate professor of Human Factors at the University of Illinois in Urbana- ‐Champaign. Dr. Wiegmann received his Ph.D. in cognitive psychology in 1992 from Texas Christian University and formerly served as an aviation psychologist and accident investigator for both the National Transportation Safety Board and the United States Navy.
Dr. Jessica P. Clark
Senior Human Factors Engineer
Dr. Jessica P. Clark received her doctorate degree from the Department of Industrial Engineering at Clemson University. Her doctoral research focused on using the HFACS framework to improve safety in the mining industry. She has experience helping companies manage their safety improvement efforts using systemic analysis techniques like the Human Factors Analysis and Classification System.
Dan McCune
Training Manager
Daniel M. McCune is currently the University Safety Officer for Embry-Riddle Aeronautical University. He is also the Chairman of the Safety committee for the University Aviation Association, and serves on several national Safety advisory committees. Mr. McCune retired from the United States Army after serving 27 years. Mr. McCune's last flying position in the Army was a Citation Jet Captain at Dobbins Air Force Base. He has flown over 6500 accident free hours. He has flown a multitude of aircraft a few are: CE-560, King Air, Queen Air, C-182, UH-1 Helicopter. Mr. McCune was the senior System Safety Engineer/Safety Director for many aviation organizations and was instrumental in designing the Army's Crew Coordination Program. Mr. McCune has a Bachelor of Arts in Business from New York University and a Master of Science in Human Resource Management from Chapman University.
Meet the HFACS Trainers
Dr. Albert J. Boquet
Trainer
Dr. Albert J. Boquet is the Department Chair of the Department of Human Factors and Systems at Embry-Riddle Aeronautical University, in Daytona Beach, Florida. Prior to that, he was a principal investigator for the Federal Aviation Administration, examining human error using the Human Factors Analysis and Classification System (HFACS). Dr. Boquet has presented and published numerous papers on the relationship between stress and the cardiovascular and neuroendocrine systems, performance in high demand environments, sleep/fatigue, and fatigue countermeasures, and human factors associated with aviation accidents. Dr. Boquet received a B.A. in psychology (1984) from Nicholls State University and an M.A. and Ph.D. in Experimental Psychology from the University of Southern Mississippi in 1991. He also completed a NIAAA post doctoral fellowship at the University of Oklahoma Health Sciences Center.
Michael Kohn
Mike has implemented HFACS in a variety of industries and is and regularly teaches HFACS internationally across a broad spectrum of businesses. He has a strong background in the Airline, Petrochemical, Entertainment, High Tech Safety, and Health industry. He has developed and created many Health and Safety Policies, procedures and training programs. Mike has analyzed over 1000 industrial accidents using the HFACS system. He is also a veteran emergency responder, having served as a Firefighter, EMT2 and certified Industrial Firefighting Instructor.
Tim Clemente
Tim is the Director of Safety for DIRECTV Home Services, where he successfully integrated HFACS into existing safety systems and processes. Prior to joining the Irvine Company, Tim was the Senior Safety Integration Manager at the Disneyland Resort in Anaheim CA. Tim’s background includes 15 years experience in commercial Nuclear Power Plant maintenance and operations. In addition, he is a commissioned officer in the US Army Reserve Civil Affairs branch with 30 years of service. Tim is a Board Certified Safety Professional as well as a Professional Member of the American Society of Safety Engineers (ASSE) and an INFRAGARD member. Tim has a Master’s Degree in Occupational Safety and Health and a Bachelor’s Degree in Business Administration. His military education includes graduation from The Command and General Staff College (CGSC/ILE), The US Army Engineer Officer Advanced Course, the Civil Affairs Officer Advanced Course and the US Army Airborne School.
